The headspace Collingwood model brings together key organisations experienced in the delivery of primary care, mental health, alcohol and drug services, social welfare and vocational education for young people. The headspace Collingwood consortium comprises membership from Melbourne PHN, Austin Health, Jesuit Social Services, Mind Australia, and St Vincent’s Hospital (Melbourne) Ltd. This strategic alliance aims to establish a community of collaborative youth services improving access for young people aged 12-25 years at risk of or experiencing issues around mental health, physical health, alcohol and other drug or vocational domains.
About the position
The headspace Collingwood Primary Health Clinic Nurse is responsible for the further development of the primary health care services delivered at headspace Collingwood for young people aged 12 – 25 years. The role will facilitate primary health screening, clinical assessment and treatment planning and delivery within scope of practice with young people; assertive follow up of results as delegated by the treating doctor; contribute to holistic care plans so that primary health is an integrated aspect of care to young people presenting to headspace; Provide support and collaborate with GP’s in the delivery of sexual & reproductive health care, information and education, immunisations, and general health follow up for young people accessing the clinic.
The position will contribute to; achieving accreditation for the headspace Collingwood (hsC) Primary Health Clinic against the RACGP standards for General Practice; development and review of audit tools, policies, procedures and evidence based clinical pathways and models of care addressing the priority needs of young people and identified diversity groups accessing headspace and collocated services.
Your key responsibilities will include
- Running of the clinic, including stock ordering within budget allocations; assertive follow up of results and treatment planning with young people; contributing to holistic care plans so that primary health is an integrated aspect of care to young people;
