National Privacy Principle 1.3 & Health Privacy Principle 1.4
YSAS collects the following personal information:
• medical information
• personal details, such as name, address, age, admission and discharge dates, billing information and Medicare information
• information generated by a health service provider, such as notes and opinions about a client and their health.
Personal health information is collected for the following purposes:
• so clients can access YSAS and be provided with support and assistance
• for use in research and planning to help YSAS provide a better service
• funding, management, monitoring, improvement or evaluation of services
• compilation or analysis of statistics in the public interest
• legal proceedings.
Access to health information
Clients are able to gain access to their health information subject to any legal restrictions pursuant to privacy law. YSAS may charge a fee as set out in Section 32 of the Health Records Act (Vic) 2001, regulation 5.
Disclosure of health information
Patients and clients must consent to disclosure of their health information to other services.
YSAS is required to submit de-identified data, which may contain health information, As part of its funding agreements with the Victorian Department of Human Services and the Commonwealth Department of Family and Community Services.
