About this organisation

The name of this organisation is Youth Substance Abuse Service (YSAS). The standard legal representation is: YSAS PTY LTD trading as Youth Substance Abuse Service

YSAS is an incorporated body and was formed in 1997 by a consortium of Centre for Adolescent Health - Women and Children's Health Network, Jesuit Social Services, St Vincent's Hospital, and Turning point Drug and Alcohol Service Inc. YSAS Pty Ltd is a company limited by shares, incorporated and domiciled in Australia.

Purposes for which this organisation collects health information

This organisation collects the following personal information:

  • Medical information;
  • Personal details, such as a name, address, age, admission, and discharge dates, billing information and Medicare number;
  • Information generated by a health service provider, such as notes and opinions about a patient or client and their health; and
  • Information about physical or biological samples, where they can be linked to a patient or client.

This organisation collects personal health information for the following primary purposes:

  • So patients and clients can access the organisation and present issues to allow staff from the organisation to provide clients with support and assistance;
  • To provide patients and clients with support and assistance, and for use in research and planning to help YSAS provide a better service. Patients or clients must consent to disclose information to other services;
  • An initial assessment diagnosis or treatment for a particular condition;
  • Further treatment and care beyond the initial assessment or treatment of a particular condition; and
  • Providing an addiction treatment program.

This organisation collects personal health information for the following actual or potential secondary purposes:

  • Funding, management, planning, monitoring, improvement or evaluation of health services;
  • Compilation or analysis of statistics in the public interest;
  • For legal proceedings;
  • Monitoring, evaluating, auditing the provision of the particular product or service our practice has, is or will provide to patients or clients;
  • Managing the provision of a health service; and
  • In order to inform a patient or client about the result of a test.

Access to health information

Patients or clients are able to gain access to their health information, subject to any legal restrictions pursuant to Privacy Law. This organisation also charges a fee set out in section 32 of the Health Records Act (Vic) 2001 and regulation 5.

Disclosure of health information

As part of this organisation's funding agreement, the Victorian Department of Human Services and the Commonwealth Department of Family and Community Services require YSAS to submit de-identified data, which may contain health information.

This organisation does not disclose health information to outside contractors.
